How do i book an appointment?
To book an appointment with your Physician or Nurse Practitioner, call (519) 633-7989 during our hours of operation.
Does CCHC Provide Emergency Services?
CCHC does not handle medical emergencies. Please go directly to your local hospital for immediate care or dial 911.
is CCHC A Walk-In?
CCHC is not a walk in clinic. To be enrolled as a patient, call the center during our hours of operation.
Do I have to pay to Recieve Services?
You do not have to pay and you do not need an OHIP card to receive services. CCHC is funded through the Local Health Integration Network by the Ministry of Health and Long-Term Care. All services are free.
What if I need to see a health professional On a Short Notice?
Every effort will be made to leave appointment time slots open and available for clients needing urgent care. Clients must call the CCHC to obtain urgent care appointments.
What if I miss an Appointment?
Clients are asked to make every effort to keep appointments and to be on time. If you cannot make an appointment, we request 24 hours notice. An appointment policy has been created to address attendance.
Can I go to another General Practioner While a client at CCHC?
When you become a client of the CCHC, you are enrolled with the Physicians and Nurse Practitioners at the centre. If you choose to see another primary care provider outside of the CCHC, the Physician or Nurse Practitioner with whom you are registered can remove you from their client list.
