Central Community Health Centre (CCHC) is a charitable, not-for-profit Community Health Centre serving St. Thomas, Central Elgin and Southwold Township.
We provide primary health care, health promotion, community programs, outreach and support through an interdisciplinary team. We work with individuals, families and community partners to improve health and wellbeing, reduce barriers to care and respond to the needs of the communities we serve.
CCHC receives base funding from the Ontario Ministry of Health to support our health-care services. Many of our community programs, outreach initiatives and enhanced services also rely on grants, partnerships and charitable donations.
Community Health Centres provide more than medical care. They bring together primary health care, prevention, health promotion, community programs and social supports to help people achieve better health and wellbeing.
CHCs recognize that health is influenced by many factors, including access to care, income, housing, food security, social connection and other life circumstances. By working as interdisciplinary teams and partnering with the community, CHCs help reduce barriers to care and support healthier, more equitable communities.

To book an appointment with your Physician or Nurse Practitioner, call (519) 633-7989 during our hours of operation.
CCHC does not handle medical emergencies. Please go directly to your local hospital for immediate care or dial 911.
CCHC is not a walk in clinic. To be enrolled as a patient, call the center during our hours of operation.
You do not have to pay and you do not need an OHIP card to receive services. CCHC is funded through the Local Health Integration Network by the Ministry of Health and Long-Term Care. All services are free.
Every effort will be made to leave appointment time slots open and available for clients needing urgent care. Clients must call the CCHC to obtain urgent care appointments.
Clients are asked to make every effort to keep appointments and to be on time. If you cannot make an appointment, we request 24 hours notice. An appointment policy has been created to address attendance.
When you become a client of the CCHC, you are enrolled with the Physicians and Nurse Practitioners at the centre. If you choose to see another primary care provider outside of the CCHC, the Physician or Nurse Practitioner with whom you are registered can remove you from their client list.
