What is Social Prescribing?
Social prescribing connects clinical care with community-based services to improve holistic health. Providers formally refer patients to local, non-clinical programs tailored to their goals, skills, and interests. This empowers individuals to learn new skills, find meaning, and stay socially connected.
For true health equity, programs must actively remove economic, geographical, and psychological barriers. Success relies on deep listening, providing continuous support, and enabling clients to co-create their own wellbeing.
