Position:  Nurse Practitioner - Part-Time - 22.5 hours per week

Job Description:

We are currently recruiting for one part time Nurse Practitioner (RN – EC). The successful applicant will work with members of our interprofessional team which includes Physicians, Nurses, Social Workers, Dietitian, Chiropodist, Health Promoter and System Navigator who work collaboratively to effectively and efficiently serve clients within the broader determinants of health framework.

As a member of this energetic team, you will work in an expanded role to provide primary health care to clients of all ages. You will also participate in case conferences, consult with and act as a resource for other team members, be an active member on many of our new internal teams and participate in quality improvement and outreach opportunities.   •          Has completed an undergraduate degree in nursing from a recognizedUniversity.
  •          Holds a Certificate of Competence from the College of Nurses ofOntario.
  •          Registered in the Extended Class with the College of Nurses ofOntario.
  •          Has minimum three years’ experience in a community health setting, or a combinationof community and hospital or public healthsettings
  •          Has experience in program development, implementation, monitoring andevaluation.
  •          Proficient in the use of computers and various software applications, including software used by the Ministry of Health and Long-Term Care, and other officeequipment.
  •          Confident working in a fast-paced environment and can respond in a positive manner to demanding issues while projecting a welcoming, friendlypersonality.
  •          Low German speakerpreferred
  •          Experience in mental health anasset
  •          Must have a valid Ontario Driver’sLicence.

Position: Chiropodist - Permanent Part-Time - 22.5 hours per week

Job Description:

CCHC is seeking a Chiropodist to perform assessment, diagnosis,treatment,health education,counseling,and other clinical footcare activities according to the College of Chiropodists of Ontario.

The Chiropodist acts as an advocate on behalf of CCHC’s target population, promoting health equity for all.

Primary Responsibilities

  •   Ensures chiropody resources, materials and equipment are maintained
  •   Contributes to chart documentation, participates in chart reviews and case conferences, and makes referrals to other health care providers where appropriate
  •   Participates in the development, implementation, monitoring and evaluation of treatment, education,counseling and health promotion programs for individuals, families and thecommunity
  •   Other dutiesas may from time to time be assigned
  •   Participates in staff development and team meetings as requested.


  •   Graduate from a recognized school of chiropody 
  •   Current registration with the College of Chiropodists of Ontario without limitations 
  •   Experience as a chiropodist in a community health or hospital setting preferred 
  •   Experience in program development, implementation, monitoring and evaluation an asset 
  •   Proficiency in the use of computers and various software applications, including software used by the Ministry of Health and Long-Term Care 
  •   Able to work in a fast-paced environment, to maintain a professional attitude at all times, to respond in a positive way to demanding issues, and to project a welcoming, friendly personality
  •   A second language is an asset
  •   Experience working with clients who have experienced trauma is an asset
