Dietitian
Permanent Part-Time – 22.5 hours per week
Wage: $32.00–$37.35 per hour, based on experience and qualifications

We are currently seeking a Dietitian to support individual nutrition care, chronic disease management, health promotion, and community-based initiatives that improve wellbeing and food security.
Nurse Practitioner – Chronic Disease Management 65+ Care Stream
Permanent Part-Time – 22.5 hours per week
Wage: $60.00–$64.55 per hour
Location: St. Thomas / Elgin County
We are currently seeking a Nurse Practitioner for a permanent part-time position of 22.5 hours per week to support our Chronic Disease Management 65+ care stream. This role is ideal for a collaborative and compassionate clinician committed to supporting older adults through holistic primary care, chronic disease management, health promotion, and prevention. At CCHC, Nurse Practitioners are supported to work to their full scope of practice as valued members of an interdisciplinary primary care team.
Registered Nurse (RN) – Foot Care and Wound Care
Permanent Part-Time – 22.5 hours per week
Wage: $42.00–$45.00 per hour
Location: St. Thomas / Elgin County
We are currently seeking a compassionate and skilled Registered Nurse (RN) to provide foot care and wound care services as part of our interdisciplinary primary care team. The Registered Nurse (RN) – Foot Care and Wound Care provides assessment, treatment, follow-up, education, and support to clients requiring foot care and wound care services. Working as part of an interdisciplinary team, this role supports clients who may be living with chronic disease, diabetes, vascular concerns, mental health and substance use challenges, poverty, trauma, and other barriers to care.
